Every entry should identify:

* effective date/version;
* affected surface;
* behavior before/after;
* migration or client impact;
* data/backfill implications;
* rollback/reconciliation notes when relevant.

Security-sensitive details describe the contract change without disclosing secrets or exploitable implementation detail.
